1. Education: High School diploma or GED required. College level courses in a medical records related field preferred, but medical records experience may be substituted. One year of management experience preferred
2. Training and Experience: Must have excellent verbal and written communication skills, good organizational skills, and the ability to effectively multitask. Must possess computer skills necessary to maintain and analyze information and to integrate and interpret data of high complexity.
3. Job Knowledge: Must have knowledge and ability to perform the essential functions of the Case Management positions. Must be able to work independently and with a team. Must be able to complete job assignments, efficiently and accurately, within set time periods without supervision. Exhibits strong interpersonal skills in order to develop and maintain relationships with patients, physicians, supporting departments, etc.

The Discharge Specialist will support the Case Management Department by coordinating the activities of the clinical and/or non-clinical areas, as well as facilitating communication among members of the Case Management team. Must utilize considerable initiative and judgment while performing duties in an area where procedures are standardized but frequent independent decisions are required. Must be able to adequately prioritize responsibilities and meet required deadlines.
Physical Effort: Must be able to lift, carry and push 50 pounds with or without assistance. Must be able to reach above head and below knees. Must be able to sit, stand and walk for prolonged periods of time.
Mental Effort: Must be able to hear, see, speak, read and write with or without accommodations. Must remain mentally alert at all times. Must be able to work well under pressure.
