Position Summary
The staff SW covers visits for the primary SWs when they need help, they are on vacation or their caseload is high. The staff SWs can see patients in any territory Unity covers (Green Bay, Oshkosh, Shawano, Sturgeon Bay, & Marinette) however, we rotate the travel to the more rural areas amongst the staff SWs. The staff SWs also assist with new admissions to the hospice program. The Social Worker provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patients and family needs. The Social Worker will participate in the provision of bereavement services as needed.
Schedule: 3 days per week, 10-hour days = 30 hours per week full-time benefit eligible. Covers one on-call weekend a month, and works 1 holiday every other year.
Tuesdays: 10 am-8 pm
Thursdays: 7 m-5 pm
Friday: 7 am-5 pm

Key Job Duties
- Comprehensively develop and continuously assess patients’ plan of care as well as the psychosocial needs of the patient, family and caregivers
- Provide supportive, educational, and crisis counseling as appropriate to patients and their families
- Engage in ongoing communication with patients, families, facility staff and team members to expedite the plan of care
- Coordinate and participate in patient and family care planning meetings
- Develop and document a thorough care plan through collaboration with the interdisciplinary team. Ensure the plan is modified based on the patient’s changing needs
- Exhibit flexibility and creative problem solving for patients with complex psychosocial needs
- Serve as a patient advocate. Enhance a collaborative relationship to maximize the patient’s and family’s ability to make informed decisions and be an active member of the interdisciplinary team
- Serve as a resource and provide education to patient, family, facility staff, physicians, and professional staff relative to the psychosocial needs of patients and families at the end-of-life
- Assess the family unit of care for bereavement risk throughout care.
- Plan for and participate in the transition from patient care to bereavement care for family members
- Foster positive internal and external customer relations, positively representing the social work profession throughout the organization and in the community
- Adhere to the Social Work Code of Ethics and consistently practice within a framework guided by ethical principles
- Prepare for and participate in interdisciplinary team meetings as scheduled
- Maintain positive relationships with contracted facilities
- Participate in community education and networking opportunities as needed
- Comply with all applicable local, state and federal laws and regulations as well as policies and procedures of Unity
- Satisfy mandatory training requirements regarding local, state and federal regulations and/or Unity’s policies and procedures
- Consistently and concisely document activity according to department protocols and standards
- Promote and maintain confidentiality
- Maintain personal and professional boundaries
- Perform other job-related duties as assigned
- Maintain regular and reliable attendance
- Shares in providing 24-hour, seven day-a-week coverage

Requirements
Qualifications
- Bachelor’s degree in social work or a Bachelor’s degree in a human services field such as sociology, special education, rehabilitation, counseling or psychology required. Master of Social Work (MSW) degree preferred
- College or university must be accredited by the Council on Social Work Education
- One year of social work experience in a health care setting
- Certified or licensed as a social worker by the State of Wisconsin
- Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage
- Internet access to accommodate charting and laptop synchronizing capabilities
