The Primary Certified Nursing Assistant (CNA) provides services and procedures that are delegated, assigned, and supervised by a Unity RN. These services and procedures are based on the RN's comprehensive assessment of the patient's personal care needs and are documented in the Activities of Daily Living Plan of Care.
This role involves traveling within the community to provide care to patients where they are-whether in their homes or in facilities. While the primary territory is Sturgeon Bay, travel outside this area may occasionally be necessary. Flexibility and a commitment to delivering compassionate care in diverse settings are essential.
Schedule : 40 hours per week - Monday-Friday 8 : 00 am - 4 : 00 pm, minimum every 3rd weekend, 1-2 holidays per year.
- This role is in the Green Bay territory but may travel outside of the area at times.
