Primary Certified Nursing Assistant (CNA)

The Primary Certified Nursing Assistant (CNA) provides services and procedures that are delegated, assigned, and supervised by a Unity RN. These services and procedures are based on the RN's comprehensive assessment of the patient’s personal care needs and are documented in the Activities of Daily Living Plan of Care.
This role involves traveling within the community to provide care to patients where they are—whether in their homes or in facilities. While the primary territory is Sturgeon Bay, travel outside this area may occasionally be necessary. Flexibility and a commitment to delivering compassionate care in diverse settings are essential.
Schedule : 4 days per week working 8 am -5 pm on Monday, Tuesday, Thursday, and Fridays (Wednesdays off - 36 hours per week). Every 8th-weekend work, on-call rotation every 8th week, 1-2 holidays per year.
Responsibilities :
Unity CNA staff can perform services / support in the following areas as directed by the RN :
- Activities of Daily Living
- Restorative Care
- Skin Care
- Mobility
- Nutritional Support
- Respiratory
- Bowel and Bladder
- Reinforce Teaching
- Supply ordering
- Companionship and Support
- Vital Signs
- Reading of CADD pump settings
- Report on pain rating
- Maintain required competencies and skills
- Report any changes in the patient’s condition or family situation immediately to the appropriate member(s) of the interdisciplinary team
