Job Summary
We are seeking a skilled and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team of healthcare professionals providing 1:1 in-home care to pediatrics and young adults with medical needs and chronic conditions in Iowa and South Dakota.

As a home health RN or LPN, you will have the chance to provide quality care and support, promoting and maintaining the health and safety of our clients while enabling them to integrate into school, family, extracurricular activities, and the community.
Key Responsibilities
- Provide skilled medical care in clients' homes, including assessments, medication administration, therapies, treatments, and interventions as scheduled in accordance with the plan of care and physician's orders.
- Partner with the interdisciplinary team, including family members, to ensure effective communication regarding client cares, changes in condition, developmental goals, etc.
- Support and educate clients and families to help them be successful.
- Build rapport and trust with clients, families, caregivers, office team, and peers to facilitate effective communication and build relationships.
- Complete timely charting through our electronic medical record system.
- Maintain up-to-date knowledge of client plans of care, changes in conditions, physician's orders, family, or caregiver requests, etc.
Requirements
- Certified as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
- Ability to work flexible schedules, including evenings, weekends, and holidays
- Strong communication and interpersonal skills
- Ability to maintain confidentiality and handle sensitive information
- Basic computer skills and ability to learn new software
