University Home Care is seeking an experienced Rn-homecare with oasis experience to join our team in Livonia, Michigan. As an Rn-homecare, you will be responsible for providing skilled nursing care to home care patients. You will work closely with patients and their families to develop and implement individualized care plans that promote their well-being and independence.
Responsibilities :
- Perform comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments
- Monitor patients' progress and make necessary adjustments to care plans
- Educate patients and their families about their health conditions and self-care
- Coordinate with other healthcare professionals to ensure continuity of care
- Document and maintain accurate patient records
