Position Summary:
Provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. Provides care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare. Facilitates discharge planning process and identifies community resources to assist patients and their families.
Coordinating dialysis patients in the ED and assist with placement for those dialysis patients presenting to the ED for urgent dialysis need.
Key Duties and Responsibilities
- Coordinates and secures outpatient dialysis chairs for patients who require hemodialysis in the home and/or clinic setting; including coordinating peritoneal dialysis treatments.
- Assist with managing the continuation of dialysis treatment in the community for complex patients
- Document as appropriate in Cerner/Impact and NaviHealth
- Enroll patients in the appropriate Up to Date (previously EMMI) program
- Monitor Outlook email ED Dialysis email group a. Respond to all referrals and communication
- AM/PM huddle with appropriate ED staff (not limited to MDs, APPs, RNs, Social Workers)
- Case manage ED Dialysis population for 90 days post placement at dialysis center a. Follow up phone calls b. Address SDOH (Social Determinants of Health) c. Schedule follow up appointments/arrange transportation if needed
- Assessment of patient medical and/or Social Determinant of Health need(s)
- Education a. Alternative resources other than utilizing the ED i. Cooper Green ii. Urgent Care iii. PCP b. Adherence and filling medications c. Appointments/follow up care d. Self-management/red flags
- Assist/connect patient with appropriate resource
- Retrieve/deliver medications from TKC Pharmacy as needed
- Assist ED staff with duties
- Perform other duties as assigned
Salary Range: $48,890 - $55,000
Bachelor's degree in Social Work required. Work experience may NOT substitute for education requirement. Currently licensed as a Licensed Baccalaureate Social Worker (LBSW) by the Alabama State Board of Social Work Examiners required.
