The Population Health Administrator applies extensive knowledge of population health, program management, data analysis, improvement methodologies to drive substantial projects of large scope and high degrees of complexity. The incumbent is accountable and plays a critical role in the development, implementation, management, and sustainment of an organization-wide strategy for maximizing quality and financial achievement in Population Health and Value Based Care initiatives. Working with the Population Health & Value Based Care team and organizational cross-functional teams, this individual will collaborate to develop the infrastructure and processes needed to meet business needs within a complex academic health system. The incumbent will exercise good judgement and creativity while independently leading a portfolio of system-level performance improvement projects from project scope and design, process redesign, implementation testing, performance metric development and on-going management for sustainment.

The Population Health Administrator is accountable and serves as a subject matter expert on overseeing programs that aim to improve health outcomes and access for certain population groups. The incumbent will demonstrate expertise in the use and application of change management methodologies and project management skills.


Your Role on the Team

Essential Job Functions:

  • PHLEBOTOMIST II_ Acts as a resource for the lab by handling and triaging inquiries to the appropriate personnel, including giving out lab reports and delivering specimens to the appropriate division. 5%
  • Lead population health initiatives which have organization-wide scope and impact. Leads cross-functional teams on broad-scope projects using advanced performance improvement models. Convenes clinical, operations, quality, and leadership stakeholders to maintain a unified and complementary strategy for quality and financial achievement on value based programs and population health platforms. Defines the process components and methodologies of the population health program portfolio. Independently creates and executes all aspects of project plans by leading, facilitating, and managing multidisciplinary improvement projects; oversees and manages multiple projects and logistics. Drives development of annual plans and proposals to outline the program structure and design to meet program goals and targets. Proactively seeks to identify circumstances that present or potentially pose a threat to value based program penalties and underperformance. 50%
  • Collaborates effectively with all levels of staff and representatives of external agencies/partners. Mentors and trains staff. Builds alliances with clinical and operational leaders in developing value based programs and systems to monitor key performance indicators. Researches and develops best practice proposals for new data collection methods and / or data systems for reporting. Serves as an expert resource on population health. Provides advice and counsel on optimal strategies and policies for performance improvement and program development in all population health and value based models. 20%
  • Analyzes and interprets highly complex data of broad scope and summarizes statistical findings. Develops corresponding reports, presentations, and action plans. Identifies information sources and oversees collection of data through a variety of mechanisms. Compares performance against benchmarks; provides proposals and plans to stakeholders to improve population health performance goals. 25%

Required Qualifications:

  • Strong knowledge of MS Office Suite applications with emphasis on Powerpoint, Excel and Visio
  • Strong analytical and critical thinking skills to effectively analyze and interpret highly complex system reports and array, identify trends, problems and potential issues, and prepare and present plans in an actionable form
  • Maintain highest standards for confidentiality of information and professionalism.
  • In-depth knowledge of Performance Improvement standards, Advance Payment Model/Value Based Care model requirements, tools, and techniques, including collection, validation, analysis, reporting of data, and use of performance and benchmark indicators.
  • In-depth knowledge and experience with population health, health program/policy analysis, and evaluating and implementing population-based health programs
  • In-depth experience with Quality, population health, and/or value-based care
  • Highly driven and self-motivated while able to work as individual contributor or as part of a team.
  • Excellent verbal and written English communication skills to compile/present a compelling story or business case
  • Excellent presentation skills, including ability to speak before large audiences and prepare and provide presentations.
  • Excellent interpersonal communication skills to communicate effectively with physicians, managers and high-level administrative staff as well as support staff who are critical to departmental functions
  • Demonstrated extensive knowledge and expertise in the use of performance improvement methodologies / tools.
  • Demonstrated ability to lead effective teams.
  • Demonstrated ability to be resourceful, innovative and take initiative to identify and search for solutions
  • Current CA RN license
  • Certification(s) in related field.
  • Bachelor’s degree

  • Ability to inspire trust and be seen as someone who provides support and assistance.
  • 5 or more years of related experience


Preferred Qualifications:

  • Prior EPIC experience
  • Master's Degree in a related field
  • Experience in healthcare setting with focused quality improvement projects
  • Demonstrated data analytics experience.
