The NAVIGATE program at M Physicians is affiliated with the University of Minnesota Department of Psychiatry and Behavioral Sciences, a leader in first episode psychosis care and research. The NAVIGATE program started in 2017 with the support of grant funds from the Minnesota Department of Human Services. Additional grant funds have been awarded to start a new NAVIGATE team in collaboration with CentraCare in St. Cloud.
NAVIGATE is an evidence-based coordinated specialty care program to treat first episode psychosis in schizophrenia spectrum disorders. This interdisciplinary team-based approach is resiliency- and recovery-focused. Team members work with individuals ages 15 to 40 years old, and their natural support persons, to help them understand their illness and develop skills that help them get back on track and live rich and meaningful lives. NAVIGATE involves a number of different interventions including medication management, individual resiliency training (IRT), help getting back to work or school (Supported Employment and Education or SEE), peer support, case management and care coordination, and family support and education. NAVIGATE team members are highly skilled at providing both clinical and non-clinical support and services to individuals and families impacted by a first episode of psychosis, whether that be in the clinic, community, or an individual's home. Team members are highly collaborative, attend weekly team meetings to discuss patient care, and receive one-to-one supervision from the program’s director.

- This role is funded through a Minnesota Department of Human Services grant, employed by University of Minnesota Physicians and works in collaboration with CentraCare in St. Cloud.**
- Attend NAVIGATE training, consultation, and supervision
- Engage in interprofessional education and collaborative care by communicating with team members in between team meetings to provide pertinent updates regarding an individual’s care
- Identifies social service needs and serves as a liaison between the patient, physician, and various community agencies providing case management-type support as needed
- Ensures all decision makers are informed of and involved in appropriate treatment planning
- Assists patients in understanding the health care delivery system and acts to resolve obstacles that prevent them from receiving adequate health care
- Assists patients and their care takers with short-term counseling following medical and mental health crisis
- Help individuals and their support persons learn about their psychosis and skills for managing
- Help individuals and their support persons identify and develop recovery goals
- Address challenges related to achieving recovery goals
- Provides in vivo training to improve individuals’ and their support persons’ coping skills
- Additional clinical responsibilities may include program referral and assessment
- Care Coordination by means of resource identification, appointment scheduling and external community collaboration
What You Will Need
- Master’s level clinician in related area.
- As a Licensed Provider: Independently licensed, eligible to be credentialed with payers; e.g. LICSW, LP, LMFT, LPCC
- As a Pre-Licensed Provider: Eligibility to work with patients or licensed at the graduate level; e.g. LGSW
- Successful background using Cognitive Behavioral Therapy (CBT) skills, including social skills training, cognitive restructuring, behavioral activation coping skills training, relaxation training and psychoeducation

- Required amount of in person work - 1 day/month in person in St. Cloud (this can include in office and/or community patient visits)
- Preferred amount of in person work - 2-4 days/month in person in St. Cloud (this can include in office and/or community patient visits)
