Position Summary | CommunityHealth Workers (CHWs) assess and address clients' social needs, developing plansto connect them with resources to mitigate Social Drivers of Health (SDOH)challenges. CHWs engage directly with Older Adults at partner organizationsites and, when necessary, in clients' homes, providing support and resourcesto improve SDOH-related adversity. Working under indirect supervision, CHWscoordinate with medical providers, primary care teams, and social services todeliver short-term care coordination and educational support, improvingclients' health and well-being. CHWs work in both clinical and communitysettings, and, when applicable, may offer peer support if they have personalexperience in mental health or substance abuse recovery. Thisposition is a Term (up to four years) training position in collaborationwith the Senior Employment Program (SEP) of the New Mexico’s Aging &Long-Term Services Department, which supports the "Health Equity and SocialConnectedness for Older Adults" programs. As part of the program, the CHWs arehired and staffed by partner organizations who receive funding support(including the cost of employing the CHW) as part of the program. It ishalf-time, twenty (20) hours per week.. Training to become a Certified Community Health Worker will beprovided as part of the program, with certification costs covered by theemploying organization. Minimum Qualifications | High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. |
