| Position Summary | Community Health Workers (CHWs) assess and address clients' social needs, developing plans to connect them with resources to mitigate Social Drivers of Health (SDOH) challenges. For this program, CHWs engage directly with Older Adults at community agency and senior organization sites and, when necessary, in clients' homes, providing support and resources to improve SDOH-related adversity. Working under indirect supervision, CHWs coordinate with medical providers, primary care teams, and social services to deliver short-term care coordination and educational support, improving clients' health and well-being. CHWs work in both clinical and community settings, and, when applicable, may offer peer support if they have personal experience in mental health or substance abuse recovery. This position is a Term (up to four years) training position through a grant funded by the Senior Employment Program (SEP) of the New Mexico’s Aging & Long-Term Services Department, which supports the "Health Equity and Social Connectedness for Older Adults" programs. It is half-time, twenty (20) hours per week, and will require the use of a personal vehicle with insurance. Training to become a Certified Community Health Worker will be provided as part of the program, with certification costs covered by the employing organization.
