Job Description
In this role, you will have the opportunity to work with a talented team of professionals who share your passion for delivering high-quality care. As a Community Support Worker, you will work closely with our multidisciplinary team to provide coordinated services and resources, promoting rehabilitation, recovery, and resiliency.
Key Responsibilities
- Individualized Care: Provide individualized care to each patient, addressing their unique needs and promoting their well-being.
- Family Support: Support family members and caregivers in understanding and navigating the healthcare system.
- Community Resources: Connect patients with community resources and services to support their recovery and resiliency.
- Documentation and Tracking: Document all patient interactions and track progress toward treatment goals.
Requirements
To be successful in this role, you will need:
- A Bachelor's degree in a related field (e.g., social work, psychology, counseling).
- At least 1 year of direct experience working with individuals with mental health conditions.
- CPR for Healthcare/BLS Provider certification within 30 days of hire.
- Ability to work collaboratively as part of a multidisciplinary team.
