Interventionist - Home Based Crisis Intervention

The Home Based Crisis Intervention (HBCI) model is an innovative service/treatment program primarily intended to stabilize clients at imminent risk of psychiatric hospitalization and assist them in gaining access to needed mental health, medical, educational, social, recreational and other services essential to meeting basic human needs. The target populations are children and adolescents (ages 5- 20 years old) with serious emotional disturbances at risk of hospitalization or residential placement requiring intensive intervention. The Interventionist is responsible for providing treatment to the child and their family/caregivers (assigned 2-3 families at a time for a 6-week period) to address the clinical needs of the child and the complex needs of the family unit. A collaborative team approach and extensive supervisory involvement are hallmarks of the program.
Responsibilities:

Provide clinical services to 2-3 families at a time, conduct up to daily meetings with each child/youth and family

Provide crisis intervention, prevention and resolution

Conduct intensive outreach to client and family

Screen and assess clients for social determinants of health, mental health, trauma, risk, and substance use

Assess client and develop service plan

Develop and facilitate service delivery

Must become fully knowledgeable of the mental health resources in Manhattan

Coordinate treatment plans with collateral service providers

Maintain complete case management records

Provide on-call rotation duties and coverage as needed

Other related duties within the scope of the position as assigned by the Supervisor
Qualifications:

MSW, or Masters in psychology or human services and licensed mental health provider or unlicensed Masters or Bachelor’s with at least one year of relevant experience in community based mental health or case management

Demonstrated ability to engage hard-to-reach populations as well as families and youth impacted by trauma, poverty, complex stressors, violence, mental health, etc.

Excellent oral and written communication skills

Experience working with children and adolescents

Comfortable traveling to and working in families’ homes and communities

Ability to be proactive and creative team player

Bilingual skills in English/Spanish, English/Cantonese or English/Mandarin is a plus. Language requirements will change based on program’s needs
Work schedule: Full time, Monday - Friday, 9:00am - 5:00pm, 35 hours per week
