What are the responsibilities and job description for the PACE Home Based Nurse position at UPHAMS CORNER HEALTH CENTER?
Job Details
This position is primarily located at participants home in the area surrounding 36 Dearborn Street, Roxbury. Home visits are to be completed to patients homes in the surrounding area.
Schedule is Monday-Friday 8am to 4pm with a rotating weekend schedule
Provide primary nursing services to participants in the home.
Provide initial and semi-annual nursing assessments;
Develop and implement nursing plan of care;
provide medication management,
Daily nursing treatments, episodic assessments and nursing interventions.
Collaborates with Primary Care to provide complete nursing services in a safe, secure, accessible environment with skill, dignity and respect.
Licensed Registered Nurse in Massachusetts
Baccalaureate Degree, Associates Degree or Diploma in Nursing
Minimum 1 year experience working with geriatric population
Self directed, organized, and flexible
Ability to speak Spanish, Cape Verdean Creole or Haitian Creole a plus
