Position Title: Orthopedic Nurse Navigator
Location: Upland Hills Health - Dodgeville Hospital Campus
Role & Department: Orthopedic Nurse Navigator in the Surgical Services Department
Hours & Shift: Part-time (.8 FTE) day shift position, Monday through Friday schedule
Role Responsibilities:
- The Orthopedic Nurse Navigator (ONN) provides compassionate and evidence-based care to patients and their families
- Will act as a leader through providing support, mentorship and educational guidance to the surgical patient through utilization of a relationship-based approach, instilling confidence in the care they will receive at Upland Hills Health
- The Orthopedic Nurse Navigator becomes the primary source of information for patients and the healthcare team
- The use of primary sources of information can improve patient experiences and ensure smooth transition through the continuum of care
- The ONN is responsible for collecting and reporting quality data and recording CMS (PROMs) questionnaires
- The ONN acts as a resource to provide education to patients and their families about the surgery and throughout the recovery process
- The ONN will communicate directly with orthopedic surgeons, other providers, and staff as part of the orthopedic care team at UHH
- The ONN will be responsible for learning the orthopedic procedures and processes provided and developed by UHH, including verbiage, diagnoses, pre-op preparation, surgery department processes, as well as post-op self-care expectations, therapy plan, and pain management
- The ONN supports the orthopedic patient by ensuring correct and timely referral to surgery, therapy, and other indicated healthcare providers and is the point of contact for patients receiving these procedures and accurately responding to questions and inquiries
- The ONN is the conduit for the orthopedic patient’s experience and positive outcome through providing accurate and timely information, referring patients to appropriate providers for any complications or questions, and will support the customer service expectations of UHH
- The ONN will be oriented to the Surgery Case Manager position and is expected to assist making Pre-op phone calls, as well as other duties assigned, to support the Surgery Department patient scheduling and pre-op preparation's
- The hours of work will typically be business hours Monday through Friday and will include coverage of Surgical Case Manager hours to ensure adequate staffing to meet patient needs and schedule volume
Qualifications:
- Graduate from an accredited school of nursing
- Licensed as a Registered Nurse in the State of Wisconsin
- A minimum of one-year acute care nursing required
- Current BLS certification
- ACLS certification or obtained within one year of hire
- Continued interest in professional growth through attendance of continuing education, in-services, participation in professional organizations, and involvement in health promotional activities
- Ability to communicate exceptionally well (verbally and written)
- Ability to plan, direct, implement, and evaluate the nursing care delivered to a variety of patients
