Job Description : UPMC Health Plan is hiring a full-time Community Paramedic to support the Connect Community Paramedics department, part of Health Plan's Community Services Team. This position is Monday through Friday business hours. Travel within the Southwestern PA region is required. The team's home office is located in Pittsburgh's North Shore; however, this position may work remotely when not traveling with some in-office attendance required, as needed. The Community Paramedic is responsible for connecting and developing a working relationship with vulnerable Health Plan Members. Introducing those members to available social influences of health resources enabling them to improve their lives. These efforts promote patient's healthy steps, develop disease management skills, ensure care transitions to avoid hospital re-admissions. The Community Paramedic performs patient biopsychosocial assessments and vital signs and assists with other services that support Community Paramedic operational efforts. Responsibilities : \n\nFace-to-face management and follow-up on individual patient caseloads using a patient-centered approach.\nComplete and document patient biopsychosocial assessments. \nConnect patients to appropriate and various social service programs to reduce gaps in care.\nConduct face-to-face, virtual, or telephonic patient interactions as appropriate.\nProfessionally and timely document all patient interactions within the Health Plans documentation system.\nPerform clinical assessments and vitals as directed and appropriate.\nComplete all training and competencies required within established timeframes.\nInteract with EMS personnel, hospital staff, and Care Management team members to coordinate patient care.\nResearch and connect patients with community resources.\nRemain flexible to future workflow changes to support program growth.\n\nHigh school diploma or equivalent required.\nCurrent (in good standing) EMS licensure with minimum 1-year experience in public health, EMS, health screening, or another health-related field.
