What are the responsibilities and job description for the Community Paramedic/EMT position at UPMC?
UPMC Health Plan is hiring a full-time Community Paramedic to support the Connect Community Paramedics department, part of Health Plan's Community Services Team. This position is Monday through Friday business hours. Travel within the Southwestern PA region is required. The team's home office is located in Pittsburgh's North Shore; however, this position may work remotely when not traveling with some in-office attendance required, as needed.
The Community Paramedic is responsible for connecting and developing a working relationship with vulnerable Health Plan Members. Introducing those members to available social influences of health resources enabling them to improve their lives. These efforts promote patient's healthy steps, develop disease management skills, ensure care transitions to avoid hospital re-admissions. The Community Paramedic performs patient biopsychosocial assessments and vital signs and assists with other services that support Community Paramedic operational efforts.
Responsibilities:
- Face-to-face management and follow-up on individual patient caseloads using a patient-centered approach.
- Complete and document patient biopsychosocial assessments.
- Connect patients to appropriate and various social service programs to reduce gaps in care.
- Conduct face-to-face, virtual, or telephonic patient interactions as appropriate.
- Professionally and timely document all patient interactions within the Health Plans documentation system.
- Perform clinical assessments and vitals as directed and appropriate.
- Complete all training and competencies required within established timeframes.
- Interact with EMS personnel, hospital staff, and Care Management team members to coordinate patient care.
- Research and connect patients with community resources.
- Remain flexible to future workflow changes to support program growth.
