Description: The coordination of transition services involves overseeing and coordinating services for the district. Responsibilities typically include working closely with students identified as having a disability in an effort to provide them with the pre-vocational, post-secondary and transition supports necessary to move from school-age to adult education, employment and/or independent living.

Qualifications:
- Valid Ohio Special Education Teachers Certificate
- Shows evidence of at least 3 years of successful teaching experience in approved programs for students with disabilities.
- Transition To Work Endorsement preferred; OR, equivalent years/experience in the coordination of transition services and supports for students with disabilities.
- Ability to identify emerging employment trends and profile training characteristics.
- Expertise in the identification and use of educational options, auxiliary services, and curricular materials that address the educational needs of program participants.
- Ability to organize community support for the school-to-career/adult living program and student assimilation activities.
- At least one of the three years teaching experience shall have been at the high school level or at an equivalent level approved by the Office of Exceptional Children (OEC).
- Meets all mandated health requirements.
- Documentation of a clear criminal record.
- Complies with drug-free workplace rules and board policies.
Essential Functions:
- Assumes responsibility for the development, evaluation and delivery of a continuum of services to meet the needs of students with disabilities which shall include, but not be limited to, community career sites, in-school work experience and paid community work experience.
- Attends IEP meetings and transition meetings.
- Works closely with parents & all school personnel in implementing work study and transition services.
- Facilitates student placements into community career sites, in-school work experience and paid community work experience (when applicable).
- Develops new relationships with community employers and maintains existing relationships.
- Maintains class lists and assigns grades for all students enrolled in the high school work- study program (copy center).
- Collaborates on individual student transition planning and suggests programs and activities.
- Provides information to students and families concerning community and adult service agencies such as OOD and FCBDD, UCBDD, DCBDD.
- Coordinates referral process for OOD.
- Works with surrounding school districts to plan & implement informational seminars for students and parents: Transition Night and College Night for Students on an IEP.
- Presents transition information to school personnel, students, parents and the community.
- Facilitates students attending Downtown or Fort Hayes Career Center: plans transition IEP meetings and oversees student progress at Downtown or Fort Hayes; maintains class lists for all IEP students attending Downtown or Fort Hayes and works closely with Downtown or Fort Hayes administrators to promote success and progress for students in career/technical programs.
- Represents Upper Arlington City Schools by joining & participating in the Ohio Association of Supervisors & Coordinators for Exceptional Children and other transition-related professional organizations (CEC; OPRA, ACTE).
- Attends conferences and informational meetings in order to provide the district with the most current information regarding transition and other work-related issues.
- Oversees Community Work Sites (maintain communication with local businesses/partnerships; participates in job-site training of students; guides classroom staff regarding training strategies; monitors data collection and student performance assessments).
- Assists in the training and supervision of paraprofessionals on job sites with students.
- Facilitates the delivery and continuous improvement of school-to-career/adult living programs.
- Complies with state model education policies and procedures.
- Upholds board policies and follows administrative guidelines and procedures.
- Promotes a favorable image of Upper Arlington City Schools.
- Builds community/school partnerships that enhance the District’s operational effectiveness.
- Helps implement the District’s continuous improvement plan.
- Collaborates with partner schools to identify, develop, and implement necessary programs and innovative services.
- Develops a strategy to accomplish personal performance objectives within specified time lines.
- Maintains open and effective communications with staff and the community.
- Keeps current with the K-12 courses of study, scope and sequence framework, and state standards/guidelines.
- Advocates for children. Helps ensure that policies and procedures support non biased assessment and program planning activities.
- Consults with Intervention Specialists regarding the academic, technological, and personal characteristics (e.g., interest inventories, job preferences, etc.) required for successful participation in anticipated learning and work environments.
- Serves as a referral agent for students transitioning or utilizing community agencies, programs, and/or services.
- Promotes a balanced perspective of work activities and family life. Develops and maintains career-focused resource materials that support teaching and student learning activities.
- Investigates options for community resources for students and families that enhance the school-to-career/adult living program (e.g., residential, recreational, transportation, health care, etc.).
- Promotes the continuity of the instructional program. Provides insights about the progression of student skills and key contributions made by staff at each level.
- Serves as a liaison and information resource for the school-to-career/adult living program.
- Consults with parents as needed (e.g., telephone calls, messages, meetings, etc.).
- Serves as a member of the IEP team; serves as a resource for Intervention Specialists in the development of appropriate/compliant transition plans including age-appropriate transition assessments, transition activities, etc. (Section 5 of the IEP)
- Supports a full range of educational options. Helps staff resolve problems related to the participation of all students in appropriate school-to-career/adult living activities.
- Provides guidance, communicates high expectations, and shows an active interest in student progress.
- Maintains high standards and upholds the student conduct code.
- Oversees the timely submission of reports, records, and inventories.
- Promotes the effective use of available technology in records management and instructional activities. Upholds computer technology acceptable use policies.
- Respects personal privacy. Maintains the confidentiality of privileged information.
- Takes precautions to ensure staff/student safety. Watches for behavior that may indicate a problem. Works with staff to eliminate unacceptable behavior.
- Reports evidence of suspected child abuse and neglect as required by law.
- Encourages and supports partner school activities as time permits.
- Participates in professional growth opportunities.
- Accepts personal responsibility for decisions and conduct.
- Ensures the safety of students.
- Establishes and maintains cooperative professional relationships.
- Remains free of any alcohol or nonprescription controlled substance and abuse of any prescribed controlled substance in the workplace throughout employment in the district.
- Exhibits professional behavior, emotional stability, and sound judgment.
- Serves as a role model for students in how to conduct themselves as citizens and as responsible, and intelligent human beings.
- Understands the legal responsibility to help instill in students the belief in, and practice of, ethical principles and democratic values.

- Occasional operation of a vehicle in inclement weather conditions, i.e., being prepared to work on all scheduled school days, except calamity days.
- Occasional requirement to lift, carry, push and pull up to a maximum of 25 pounds.
