Job Description
The Care Specialist will work closely with patients, families, and caregivers to develop and implement personalized care plans. They will also collaborate with an interdisciplinary team to achieve market and/or organizational goals.
Key Responsibilities
- Conduct direct outreach to patients via phone, in-person, mailings, and other strategies.
- Enroll patients into Upward Health's program and collect key data during the enrollment process.
- Meet with patients in their homes or community settings to conduct needs assessments, set health goals, and coordinate care with the care team.
- Facilitate in-home provider appointments and ensure that all assigned patients have initial provider visits and follow-up appointments scheduled.
