What are the responsibilities and job description for the Healthcare Advocate position at Upward Health?
Key Responsibilities:
- Conduct direct outreach to patients via phone, in-person, mailings, and other strategies
- Enroll patients into our program and collect key data about patients during the enrollment process
- Meet patients in their homes or in the community to conduct needs assessments and help patients set health goals
- Coordinate care between patients and interdisciplinary care teams as needed
- Support patients in meeting their healthcare goals as it relates to quality measures
In addition to these responsibilities, you will also need to have the ability to analyze patient data, assess physical conditions of patients, explain technical medical information to educate patients, and cultivate relationships with primary care providers.
