Job Description
We're seeking a skilled Home Health Care Coordinator to join our team. As a Home Health Care Coordinator, you'll work in patient's homes and community 90% of the time and virtually the other 10%. Your primary responsibility will be to deliver chronic care management to high-complexity patients.
You'll use a variety of strategies, including phone calls, in-person visits, and mailings, to engage with patients and help them achieve their health goals. This may involve conducting needs assessments, developing care plans, and coordinating with other healthcare professionals.
