Job Summary:
We are seeking a highly motivated Community Health Advocate to join our team. As an Outreach Specialist, you will be responsible for conducting direct outreach to patients, communicating our service offerings, and enrolling patients into our program.
Your Key Responsibilities:
- Conduct direct outreach to patients via phone, in-person meetings, mailings, and other strategies
- Communicate with patients about the role Upward Health can play in improving their health
- Enroll patients into our program and collect key data during the enrollment process
- Manage a personal caseload of patients and ensure each assigned patient is contacted using the most effective strategy
- Interact with enrolled patients as needed to ensure ongoing engagement or deliver additional community-based touchpoints
- Participate in weekly team meetings focused on education and improvement
- Accurately document interactions in our company's software system
