About the Position
We are seeking a dedicated Care Specialist to join our team at Upward Health. As a Care Specialist, you will play a critical role in delivering high-quality care and support to patients with complex needs.
The successful candidate will be responsible for conducting direct outreach to patients, enrolling them into our program, and coordinating care between patients and interdisciplinary teams. You will also work closely with primary care providers to ensure seamless continuity of care and data integrity.
Requirements:
- At least 2 years of relevant work experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or similar role.
- High school graduate or GED required.
- A valid driver's license and auto liability insurance.
- Reliable transportation and the ability to travel within your assigned territory or other as needed.
