About the Position
We are seeking an experienced and empathetic Patient Engagement Specialist to join our team at Upward Health.
In this critical role, you will work directly with patients to assess their needs, develop personalized care plans, and coordinate care with our interdisciplinary team. Your primary focus will be on promoting timely access to services, reducing emergency room utilization and hospital readmissions, and supporting patients in achieving their health goals.
Key Responsibilities
- Conduct home visits, cold calls, and other outreach activities to engage patients in their care and provide education about our services.
- Enroll patients into our program and collect key data about their needs and preferences through ongoing interactions.
- Analyze patient data to determine their needs or treatment goals, and assess physical conditions to aid in diagnosis and treatment.
- Explain technical medical information to patients in a clear and concise manner, and facilitate communication between patients and healthcare professionals.
- Maintain accurate and timely documentation of patient information in electronic medical records to ensure seamless continuity of care and data integrity.
Requirements
- At least 2 years of experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or similar role.
- High school graduate or GED required.
- A valid driver's license and auto liability insurance.
- Reliable transportation and ability to travel within assigned territory or other locations as needed.
- Experience in Chronic Care Management model or working with chronically ill/elderly patients preferred.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
