Job Description
The role of a Clinical Social Worker at Urban Health Plan involves working as a psychotherapist within our outpatient primary care health centers. As a part of our Behavioral Health Services Division, you will engage with children and/or adults and families to promote their overall health and wellness through evidence-based clinical therapy.
Your responsibilities may include conducting individual psychotherapy sessions, crisis intervention when necessary, referring patients or family members to community resources, and providing consultation to medical providers.
