What are the responsibilities and job description for the Discharge Planner position at Urban Recovery House LLC?
A discharge planner is a person who functions as a consultant for the discharge planning process within the facility providing education and support to clients in the development and implementation of discharge plans.
Responsibilities:
- • Effectively provides clinical and psychosocial services to patients, involving patients’ support systems including family, friends, counselors, and treatment providers. • Educates, coordinates, and collaborates with nurses, physicians, counseling staff and other interdisciplinary members of the team, to assure ongoing comprehensive care. • Dependably coordinates and facilitates the development of individualized treatment plans and actively participates in interdisciplinary treatment team meetings. • Provides thorough and accurate discharge planning information for initial treatment plans during interdisciplinary treatment team meetings. • Routinely and professionally collaborates with external agencies and providers of service in such areas as mental health and substance abuse to implement an effective, safe and individualized discharge plan. • Coordinates and facilitates safe and appropriate discharge plans for patients which include case management referrals and follow-up with outpatient appointments addressing patients identified needs. • Routinely provides psychosocial education to patients and families as scheduled and as indicated in treatment plans. • Responsible for coordinating referral follow-up and coordinating with post discharge referrals to confirm patients’ next level of care.
• Performs functions related to effectively and efficiently assisting patients through smooth, safe, and quality transitions for aftercare. - Establishes and maintains relationships with community providers, referral sources, hospitals, and social support.
• Ability to research and create linkages based on patient needs.
• Effectively audits and insures the implementation of Individualized Treatment Plans, Psychosocial Histories and Integrated Summaries within established time frames. • Continuously maintains compliance with program standards. • Consistently maintains compliance with staff development training. • Completes all outcomes measures thoroughly and accurately. • Regularly and safely assists with de-escalation of agitated patients utilizing approved techniques.
• Facilitate crisis invention approaches to engage patients in treatment activities.
• Assist with therapeutic programming around group engagement. • Routinely demonstrates a sound understanding of and commitment to complying with OASAS program standards. • Consistently displays cultural competence and sensitivity and ensures services provided incorporate patient-specific cultural values. • Successfully attains/maintains competency with company electronic health record and uses it effectively within required time. • Reliably completes other related duties as assigned.
