Overview:
The Hospice Liaison is responsible for facilitating and fostering relationships with healthcare providers, hospitals, and community organizations to promote the hospice program and services. The liaison will serve as a key point of contact for referrals, educate healthcare professionals and families about hospice care, and ensure a seamless transition of patients into the hospice care.
This role requires a compassionate and knowledgeable individual who can effectively communicate the benefits of hospice care and ensure the highest quality of service for patients and their families.
Key Responsibilities:
Referral Development & Management:
- Develop and maintain relationships with local healthcare providers, including physicians, hospitals, nursing homes, and community organizations to generate patient referrals.
- Respond promptly to incoming referrals and assess patient eligibility for hospice services.
- Collaborate with the admissions team to ensure a smooth and timely transition of patients to hospice care.
Community Outreach & Education:
- Educate healthcare professionals, families, and the community about hospice services, end-of-life care, and the hospice philosophy.
- Conduct in-service presentations, workshops, and training sessions for healthcare staff and community groups.
- Act as an ambassador for hospice care, advocating for patients and families in various healthcare settings.
Patient & Family Support:
- Provide emotional and informational support to patients and families as they consider hospice care.
- Assist families in understanding the benefits of hospice care, the services provided, and the process of enrollment.
- Ensure that all inquiries and concerns from patients and families are addressed in a timely and compassionate manner.
Collaboration with Internal Teams:
- Work closely with the hospice clinical team, including nurses, social workers, chaplains, and physicians, to ensure that patient needs are met.
- Participate in team meetings and contribute to the care planning process for referred patients.
Compliance & Documentation:
- Ensure all necessary documentation is completed accurately and in a timely manner.
- Adhere to all federal, state, and local regulations related to hospice care, including HIPAA and Medicare guidelines.
- Maintain confidentiality and privacy of patient information.
Qualifications:
- Bachelor’s degree in nursing, Social Work, Healthcare Administration, or a related field Preferred but not required. In lieu of a degree equivalent work experience will be considered.
- Experience:
- Minimum of 2 years of experience in hospice care, case management, healthcare sales, or a related field.
- Experience in community outreach or healthcare liaison roles is highly preferred.
Skills:
- Strong communication, interpersonal, and presentation skills.
- Ability to work independently, as well as part of a team.
- Excellent organizational and time-management skills.
- Knowledge of hospice care, palliative care, and end-of-life care principles is a plus.
Certifications:
- Valid driver’s license and reliable transportation.
- Ability to travel locally for community outreach and healthcare provider visits.

Pay: $70,000.00 - $125,000.00 per year
