POSITION: LPN Navigator

LOCATION: Cincinnati, OH

WEEKLY HOURS: 40 (Full time)

OHC - Specialists in Cancer and Blood disorders, is seeking an LPN Navigator to work between our Blue Ash and Mercy West locations. As an LPN Navigator, you will be responsible for providing individual assistance to patients, families, and caregivers to overcome healthcare system barriers and provide timely access to quality healthcare within a community oncology setting. Regular and frequent interface with practice-based leadership teams, physicians, and clinical staff is required to promote and engage the patient navigation process. As the only independent adult cancer program in the region, we offer Medical, Radiation, Blood and Marrow Transplant, Gynecologic, and Breast Surgical Oncology services. OHC supports a robust cancer research and clinical trials program, neuro-oncology, cancer genetics specialists and supportive care program.

Responsibilities

  • Provides coordination of care during the full continuum of cancer care experience.
  • Observes, records, and reports patient's condition and reaction to drugs and treatments to physicians.
  • Screens patients for appropriate information. Instructs patients in collection of specimen samples for testing.
  • Responds to and refers incoming telephone calls. Instructs patient and family regarding medications and treatment instructions.
  • Phones in prescriptions and makes other patient calls as required or directed by physicians.
  • Maintains patient files, records, and other information.
  • Participates in team huddles led by the ONN and responsible to disseminate information to the team.
  • Provide education materials.
  • Supports patient advocacy and empowerment
  • Participates in community outreach and cancer prevention education

Qualifications

MINIMUM QUALIFICATIONS:

High school graduate or equivalent. Graduate from an accredited school for practical nursing. This position requires at least 5 years of experience as a practical nurse. Current LPN state license and registration with the State Board of Practical Nursing in practicing state. Current BCLS certification required within 3 months of hire.

standing and walking for extensive periods of time. Occasionally lifts and carries items weighing up to 40 lbs. Requires corrected vision and hearing to normal range.
