What are the responsibilities and job description for the Nurse Case Manager II position at US Tech Solutions?
Location: Remote with Travel in Fulton, Lucas, Ottawa, and Wood Counties, OH
Duration: 6-Month Contract
Responsibilities:
- Coordinate case management activities for Medicaid Long-Term Care/Comprehensive Program enrollees.
- Apply critical thinking and judgment to support the case management process, ensuring optimal healthcare outcomes for members.
- Provide care coordination, support, and education using case management tools and resources.
- Conduct comprehensive member evaluations utilizing data and care management tools.
- Develop, implement, and monitor individualized care plans, ensuring progress toward healthcare goals.
- Conduct multidisciplinary reviews to achieve optimal health outcomes.
- Identify and escalate quality-of-care concerns through established protocols.
- Utilize negotiation skills to secure appropriate services and benefits for members.
- Apply motivational interviewing techniques to maximize member engagement and encourage positive health behaviors.
- Provide coaching and education to empower members in making informed healthcare decisions.
- Facilitate active member participation in their healthcare plans in collaboration with providers.
- Ensure compliance with regulatory and accreditation guidelines through case management and quality management processes.
Qualifications:
- Required: Case management experience.
- Preferred: Experience in long-term care.
- Proficiency in Microsoft Office, including Excel.
Education & Certification:
- Active and unrestricted Registered Nurse (RN) license in the state.
- Preferred: Certified Case Manager (CCM) certification.
