What are the responsibilities and job description for the Nurse Case Manager (LPN) position at US Tech Solutions?
Duration: 3 Months
Location: Toledo, Ohio (Requires 50-75% travel using personal vehicle for face-to-face member visits)
Job Responsibilities:
- Conduct phone-based interactions with low-stratification members, coordinating care, completing, reviewing, and updating assessments and care plans that address problems, goals, and interventions.
- Develop and maintain individualized care plans based on assessments and claims data.
- Participate in Interdisciplinary Team (IDT) meetings to discuss members’ healthcare needs, identify barriers, and explore opportunities for improved outcomes.
- Identify and connect members with health plan benefits and community resources.
- Perform administrative tasks to maintain necessary skills for job duties.
Key Duties:
- Manage care for members enrolled in the Dual Special Needs Plan (D-SNP), typically categorized as low-to-medium stratification or those with Social Determinants of Health (SDOH) needs.
- Collaborate with Nurse Care Managers, Care Coordinators, Transition of Care (TOC) Coaches, and other team members to enhance health outcomes and connect members with community-based organizations.
- Assess members’ needs and care gaps, communicate with Primary Care Providers (PCPs), maintain updated care plans, and participate in interdisciplinary team meetings.
- Identify cases that require clinician involvement and ensure appropriate transitions.
- Complete health screening questionnaires and market-specific assessments.
- Support outreach efforts for members who are difficult to reach via telephone or in-person visits.
- Ensure members receive and understand their prescribed medications.
- Manage a caseload of members with varying levels of care needs in compliance with contract guidelines.
- Provide clinical support, including prior authorization coordination, specialist referrals, condition management education, medication management, and connections to community resources.
Qualifications & Experience:
Required:
- Licensed Practical Nurse (LPN) with a minimum of 2 years of nursing experience (3 years preferred).
- Proficiency in computer systems, including Microsoft Office Suite.
Education:
- High school diploma or equivalent required.
- Active LPN license required.
