Pay Range: $28.34 - $42.52/ hour
Additional components of compensation may include:

Per Diem Staff will not be eligible to participate in any associate benefit plans sponsored by the Health System.

POSITION SUMMARY: Reviews all OASIS time points for accuracy and supporting documentation. In collaboration with other members of the Health Care Team, assists with the planning, development and implementation of the Quality Improvement Plan and Program for all OASIS time points within the Agency incorporating principles of QI and UR and ensuring compliance with all accrediting agencies. Coordinates Utilization and reviews activities and assists with data collection process. Participates in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as Health System initiatives. |
EDUCATION/TRAINING: Completion of an approved Registered Nursing Program. BSN preferred. At least one year clinical experience in Home Health Care required. |
LICENSURE/CERTIFICATION: Current Registered Nurse licensure in the State of Illinois required. Must have valid State driver's license and proof of liability insurance. |
EXPERIENCE/SKILLS/ABILITIES: Successful completion of annual job specific competencies and skill validation tools. Good interpersonal skills and ability to relate well with patients, family members, physicians and all levels of staff within the organization and resources within the community. Excellent verbal, written, organizational and critical decision-making skills required. Demonstrates leadership and problem solving skills. Ability to observe Home Health employees at patient homes. Proficient in MS Word, Excel, and Power Point. |
