60% FTE, day / evening shift. On-call responsibilities. Weekend and holiday rotation. This position is eligible for shift and weekend differentials. Hours are based off the operational needs of the department. You will travel and work primarily in patient's homes, your assigned work location will be 301 S. Westfield Rd., Madison, WI.

Exciting opportunity to join our Home Based Hospital Care team providing hospital level care in the comfort of the patient's home!
We are seeking a Registered Nurse (RN) to :
- Provide hands-on acute care in the home setting for the Home-Based Hospital Care patient population requiring the use of advanced assessment, technical, teaching, and decision-making skills. Other skills utilized may include : IV administration, lab draws, medication administrations, wound care, utilization of remote patient monitoring and video visit communication with the patient / family.
- Work directly with patients and families in the comfort of their home utilizing care plans, interventions and equipment that are modeled after an acute care inpatient unit setting.
- Work in collaboration with the interdisciplinary team to ensure that patients receive exemplary care in their home.
- Perform comprehensive assessments of physical, functional, psychosocial, and cognitive status of the Home-Based Hospital Care patient utilizing interview observations and physical exam techniques.
- Assess post-acute needs and ensure that appropriate referrals to other services are made. Interdisciplinary conferencing is a key component of this role's responsibility, and thorough and timely documentation in EPIC is required.
