Job Description :

Our Care Coordinator DCS provides care to patients under DCS guardianship by supporting patients and families in navigating systems and advocating for their needs. The DCS Care Coordinator will function as a trusted liaison between external departments and families. DCS Care Coordinators will manage caseloads of patients under DCS guardianship and provide weekly outreach focusing on connecting families to resources, facilitating wrap around care, and supporting families in accessing high needs services. Additionally, DCS Care Coordinators also adopt an integrated approach when managing cases that involves collaboration with a team of clinicians and providers as well as frequent staffings to ensure that total wellness is considered throughout treatment. As a managing clinician on the case, the DCS Care Coordinator will also conduct annual assessments, screeners, and service plan updates to ensure that the patient and family's needs are captured within the identified goals and services that are provided. DCS Care Coordinators will also facilitate monthly CFTs to ensure that patients and families are progressing toward treatment goals.

Salary : $43,680 / annual
