Job Summary
The Client Care Coordinator plays a pivotal role in supporting clients through the intake, placement, and aftercare processes within the program. This position ensures clients receive the necessary resources and support to achieve their recovery and reintegration goals. The Client Care Coordinator is responsible for initial client assessments, insurance benefit management, and effective coordination with clinical teams, ensuring smooth transitions and quality care for clients.
Responsibilities :
Facilitate and document group sessions, focusing on recovery, life skills, and other relevant topics.
Conduct initial assessments for incoming clients, gathering detailed information to determine appropriate clinical placement.
Staff with the clinical team to ensure correct placement based on assessment results and program phases.
Verify active AHCCCS insurance benefits for clients.
Assist individuals without active benefits by reactivating or applying through HEA or other platforms.
Collaborate with clinical and administrative teams to ensure alignment in treatment and care planning.
Assist the clinical team in training, providing direction, and ensuring compliance with organizational processes.
Monitor and document aftercare plans for clients, ensuring plans are realistic and actionable.
Track progress and maintain records related to aftercare and follow-ups.
Distribute and collect client satisfaction surveys to evaluate program effectiveness.
Provide feedback to management based on survey results to enhance program quality.
Undertake additional tasks as assigned by management to support organizational goals.
Requirements
Required Skills / Abilities :
Must be proficient in computer applications and electronic medical record systems.
Proficient with Microsoft Office Suite or related software.
Some assignments require an Arizona Fingerprint Clearance Card
Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).
Education and Experience
Meets one of the following State requirements :
Has a bachelor's degree and at least one year of full-time behavioral health work experience;
Has an associate's degree and at least two years of full-time behavioral health work experience;
Has a high school diploma or high school equivalency diploma and : 18 credit hours of post-high school education in a field related to behavioral health completed no more than four years before the date the individual begins providing behavioral health services and two years of full-time behavioral health work experience; or
Four years of full-time behavioral health work experience :
Minimum one year of case management knowledge and experience with the AHCCCS population, justice involved persons, chronically underhoused and the substance use treatment.
Required Competencies :
Entry-Level Clinical Knowledge- Demonstrating an understanding of basic clinical concepts and practices to support client needs and team objectives.
De-escalation - Uses appropriate interpersonal styles and techniques to reduce tension and / or conflict between two or more people; able to size up situations quickly; able to identify common interests; facilitates resolution.
Clinical Evaluation- Identifying and connecting clients to appropriate services, including SMI evaluations.
Trauma Informed Client Care- Demonstrates the ability to adapt approach based on client's various levels of trauma. Makes clients and their needs a primary focus of one's actions; gains client trust and respect; meets or exceeds client's expectations.
Planning and Organizing - Organizes work, sets priorities, and determines resources requirements; determines necessary sequence of activities needed to achieve goals.
Adaptability - Adapts well to changes in assignments and priorities; adapts behavior or work methods in response to new information, changing conditions, or unexpected obstacles; approaches change positively and adjusts behaviors accordingly.
Collaboration & Teamwork- The ability to function effectively as a member of an inter-professional team that includes behavioral health and primary care providers, and clients. Participates as an active and contributing member of a team to achieve team goals. Works cooperatively with other team members, involves others, shares information as appropriate, and shares credit for team accomplishments.
Documentation - Knowledge of regulations. Prepare accurate, concise notes. Write comprehensive, clear psychosocial narrative. Record client progress in relation to treatment goals. Appropriate verbiage. Discharge summaries.
