Job Description :
The Integrated Community Health Worker (ICHW) is an essential role that supports patients by connecting them to appropriate Behavior Health and Primary care services and resources, provides individualized care coordination for patients with unmet, complex, or ongoing medical needs to ensure communication between all providers regardless of their discipline, follows up with patients to help them overcome barriers to successfully implement their action plan or treatment recommendations as needed, and monitors the accuracy of all aspects of the patient's chart as stated on the Integrated Service Plan (ISP). The ICHW will become trained in and lead interventions on Chronic Disease Self-Management, will be responsible for connecting with patients and coordinating with staff to maintain documentation compliance and service delivery, and must have strong organizational and administrative skills to support monitoring expiring and inaccurate documentation.
