Memory Support Program Manager

Valle Verde, a HumanGood community in Santa Barbara, recently named "Best Continuing Care Retirement Community in the US - 2024 and 2025 by Newsweek magazine, is hiring a Memory Support Program Manager ("MSPM"). The MSPM coordinates and leads the community' s memory support program to ensure that the physical, social, and emotional needs of the residents and their families are met. S/he also promotes and engages team members and residents in "Best Life" philosophy.

What is Best Life? Best Life is a unique and holistic dementia care approach that helps people and organizations support those living with dementia to live rich and rewarding lives!

** Salary Range: $60,000 - $75,000/year (DOE) ** Work Schedule: Monday- Friday

Essential duties and responsibilities of the position include, but are not limited to:

• Keeping the HumanGood Memory Support Program Philosophy at the core of all planning and implementation of activities and care to meet the specific needs of residents
• Collaborating with Clinical partners to coordinate resident moves and transfers within the Memory Support department
• Assure that resident profile is completed (prior to move-in preferable; no later than 3 business days after move-in) and shared with team members;
• Conducting orientation and promoting support and/or educational groups for residents and family members
• Coordinates with Director of Wellness and Assisted Living when significant changes of residents occur which may lead to level of care change or adjustment;
• Preparing and updating Level of Care Transfer Notices and Service Plans for Memory Support residents in collaboration with the Director of Wellness and Assisted Living
• Maintaining readiness for licensing and accreditation surveys
• Completing required state training programs for Memory Support
• Performing other critical duties and responsibilities as required

To be successful in the role, you would have

• Bachelor's degree in social work, recreation therapy, aging, business, or related field, preferred.
• Three to Five years of prior related experience or any equivalent combination of training and experience which provides the required skills, knowledge and abilities.
