FAMILY CONSULTANT
CASE MANAGER (Fresno)
POSITION SUMMARY
Under the direction of the Program Manager, the responsibility of the Family Consultant is to provide consultation, care planning assistance and support services to caregivers and the families. Serve the population of brain-impaired or frail, elderly adults, in accordance with Valley Caregiver Resource Center (VCRC) policies, procedures and contract requirements.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Consult with family caregivers to perform care planning consultation, education and support services.
- Conduct an assessment of needs, home visits and develop short-and long-term care plans.
- Oversee respite care services, provide referrals to other community agencies with the authorization of appropriate VCRC voucher services.
- Facilitate caregiver support groups.
- Organize and participate in educational programs as needed.
- Ensure compliance to CRC services standards and protocols.
- Represent the agency through public speaking engagements, training activities and community outreach efforts.
- Collect data and carry out activities as required by grantors/grants and report grant related activities to the Program Manager.
OTHER RELATED DUTIES
- Attend meetings as requested.
- Participate in annual fundraising event(s).
- Ensure and maintain a safe, professional work environment.
- Complete other projects and duties as requested.

QUALIFICATIONS
Education
- Bachelor’s Degree or equivalent experience in areas such as Social Work, Psychology, Sociology, Counseling, Gerontology or related fields preferred.
Skills and Experience
- Minimum 2 years’ experience in Case Management or related fields preferred.
- Bilingual skills strongly preferred.
- Knowledge of counseling and social services principles and practices helpful.
- Ability to speak publically, educate, train and understand the unique needs of caregivers, the elderly and families.
- Ability to prepare and present reports in a clear and concise manner.
- Ability to perform needs assessments, provide solutions and resources for a variety of family needs.
- Ability to demonstrate good judgement in providing resources to a diverse group of clients.
- Ability to manage and prioritize multiple tasks under deadline pressure.
- Highly organized and accurate with attention to detail in all areas of work.
- Ability to maintain confidentiality of VCRC, staff and client information.
- Strong skills in Microsoft Word, Excel and Outlook.
- Strong electronic, verbal and written communication skills with supervisor, employees, clients and community members.
Licenses and Certificates
- Must possess a valid California Driver License and insurance is required.
- Must be willing to travel locally and out of town.
- Completion of a successful background check will be required.
