Job Summary:
The Case Manager is responsible for delivery of case management services to all assigned clients and works in collaboration with the care team to provide ongoing care coordination and treatment planning. The Case Manager will work collaboratively with clients and their families to develop a medically necessary treatment plan, utilizing the concept of the Golden Thread, DLA-20 assessment, and Motivational Interviewing to help guide treatment planning efforts. The Case Manager will be responsible for supporting clients in developing skills to improve functioning and independence in activities of daily living as well as link clients to appropriate community-based services and support. This position will be primarily based in the office, however at times may be required to complete outreach in various settings within the community.
Key Responsibilities:
- Meet with new clients and their families to explain the model of care and provide a brief introduction to the services available at VCC.
- Conduct the DLA-20 Assessment to determine case management goals and collaborate with clients and their families to create and maintain a medically necessary treatment plan in accordance with VCC procedures.
- Provide case management and care coordination services as indicated by the treatment plan and supportive to the identified level of care and need for the client.
- Support clients in developing skills to improve functioning in activities of daily living (personal hygiene and grooming, nutrition, grocery shopping, budgeting, maintaining stable housing, etc.).
- Achieve 60% productivity (24 hours) billable services per full FTE.
- Assist in identifying and linking clients to appropriate community-based services and support (primary care, mental health and/or substance abuse treatment, housing, financial benefits, etc.).
- Facilitate/co-facilitate educational and/or skill building group treatment services to support clients in learning skills to manage chronic health conditions or improve functioning in daily living activities.
- Provide crisis intervention as needed.
Minimum Qualifications:
Education:
- BA Degree in Behavioral Sciences field.
- One-year clinical experience.
Experience:
- Crisis response, verbal de-escalation
- Treatment planning
- Demonstrated ability to work effectively with a variety of professionals and a diverse client population with a strengths-based approach.
