The Care Transitions/Daytime Crisis Specialist Hybrid will be responsible for coordinating hospital referrals for LRO monitoring. They will monitor and provide services for individuals on less restrictive orders (LROs) to support increase engagement in outpatient services with a focus on reducing or preventing more acute illness, re-hospitalizations, and other emergency medical or crisis responses. The Care Transitions/Daytime Crisis Specialist Hybrid will also answer the daytime crisis phone line and respond to crisis calls. This position will include both engagement and crisis outreach to clients on LROs and/or who have been identified as in crisis.
Essential Job Functions (this is not the job description in its entirety):
- Participate in continuity of care and discharge planning with hospitals (emergency departments, inpatient psychiatric, etc.), Valley Cities care team, DCRs, and other community partners to ensure continuity of care and support treatment goals.
- Monitor and provide services for individuals who have been conditionally released from psychiatric hospitalization into outpatient services
- Provide telephonic crisis support services including risk assessment, stabilization, and referral.
- Provide outreach services as needed to improve engagement and/or assess for danger to self, danger to others, and grave disability.
- Demonstrate clinical proficiency in evaluating individuals for danger to self, danger to others and grave disability
- Other duties as assigned
Minimum Qualifications:
Education:
- Master’s degree in the social or behavioral sciences.
- MHP Preferred, meeting King County and Washington State Mental Health Professional (MHP) requirements as set forth in WAC 388-865-0150; RCW 71.05.020(27) and RCW 71.34.020(14) or
- Valley Cities employee with a bachelor’s degree in the social or behavioral sciences who has demonstrated proficiency with the required skills
Experience:
- Working with individuals with multi-system involvement and requiring intensive supports
- Crisis assessment and response
- Safety planning over the phone and in-person
- Experience working in hospital and/or jail settings (preferred)
