Job Summary:
Functions as an integral part of a multidisciplinary team under the direct supervision of the Chief Operations Officer. The Director of Community Outreach Programs is responsible for the delivery of services in all of VC’s community outreach programs This position also has oversight of the agency’s daytime and after hours crisis outreach and diversion functions, and transitions of care within the agency The intent of the care model is to facilitate transitions and linkages to ensure optimal health outcomes and seamless care along the healthcare continuum in a variety of specialized programs providing outreach services in the community to meet clients where they are at. The Director should have knowledge of chronic care management, hospital systems, managed care, health home, crisis and diversion, care transitions, and fully integrated managed care systems.
Key Responsibilities:

Key Responsibilities:
- Management/Administrative Responsibilities
- Supports the Managers who oversee After Hours Crisis, Daytime Crisis, Care Transitions, New Journeys, Mobile Van Clinic, Veterans Services, WISE /Wraparound, Employment Services, Assisted Outpatient Treatment, and the Prevention and Community Support Program.
- Responsible for the general leadership and staff development of the assigned programs to accomplish and support VC’s missions, goals and objectives.
- Institutes and maintains an organizational vision and environment that emphasizes teamwork, trust, professionalism, efficiency, and customer service
Financial Responsibilities:
- Prepares an annual budget for the Chief Financial Officer’s review and approval that follows and supports the mission and strategic plan of VC
- Uses cost benefit analysis as a tool for decision making
- Submits data to help prepare VC’s financial reports
Human Resources Responsibilities:
- Effectively manages employee conflicts, and builds and maintains a sense of teamwork among staff members
- Recruits and selects staff in consultation with site team, medical director, behavioral health directors, HR, and/or other clinical services directors
- Places high value on cultural competency and ensures that hiring reflects community demographics
Minimum Qualifications:
Education
- Master’s degree in behavioral sciences or human Services field.
- Washington state licensure as a LMHC or LICSW or LMFT required.
- Mental Health Professional required.
- Demonstrates a clear understanding of the principles of recovery and value of peer support
- Training in utilization management, care transitions across the continuum of health systems
- Knowledge of chronic care management, hospital systems, managed care, health home, crisis and diversion strategies
- Ability to learn Fully Integrated Managed Care and Washington State Medicaid Transformation initiatives
Experience:
- Demonstrate progressive responsibility and significant experience in business administration, particularly management of financial systems and human resources.
- Minimum five years supervisory experience.
- Minimum five years of experience in a health or human services field.
- Can illustrate documented, service-oriented experience that includes successful contacts with political representatives, community and business leaders, contributors, and public at large.
- Proven success in written communications.
- Grant writing skills desirable but not required.
