VCSB is looking for a Permanent Support Housing (PSH) Mental Health Case Manager! The PSH Case Manager provides ongoing mental health case management to PSH participants experiencing severe mental illness (SMI).

Summary of Essential Functions
- Collaborates with PSH Specialist to promote housing stability.
- Provides comprehensive initial and annual assessment of client strengths and needs.
- Participates in development of the consumer service plan.
- Refers, links, and coordinates Medicaid and non-Medicaid services.
- Provides supportive counseling and case management services.
- Provides crisis intervention and crisis stabilization services as needed.
Minimum Qualifications
All applicants must meet the following minimum qualifications in order to be considered qualified for the position:
- QMHP certification, and Bachelor's degree.
- One year’s clinical experience with adults with SMI, and
- Valid Virginia driver’s license with an acceptable driving record.
