The Community Participation Supports Specialist is responsible for the care of individuals with intellectual disabilities in a community setting. The CPS Specialist is assigned to specific individuals for whom they are responsible for providing services based off the specified goals in their plans while in the community. These responsibilities are exercised in a manner that assures the greatest amount of dignity, privacy, and care possible for the individual and involves the individual in decisions regarding their continued supports and services. An agency vehicle is provided for use during working hours. The CPS Specialist will work in the Slippery Rock, PA area.
Essential major duties and responsibilities include, but are not limited to:
- Assists individuals in acquiring, maintaining, and improving self-help, domestic, socialization and adaptive skills necessary to reside successfully in home and community-based settings
- Support individuals in general areas of self-care, communication, fine and gross motor skills, mobility, therapeutic activities to acquire, improve or maintain skills, personal adjustment, relationship development, socialization, and accessing and utilizing community resources as detailed in the ISP (Individual Support Plan)
- Ensures the health, safety and welfare of individuals by providing supervision and reporting concerns and absences
- Attends required staff meetings
- Attends required training programs
- Completes and submits all required documentation including time cards, progress notes, incident reports, mileage sheets, individual records and other required documentation
- Reports significant changes in health, safety, or behaviors by reporting both verbally and in writing
- Coordinates with the individual and team members on an on-going basis in a professional and appropriate manner
- Is expected to be fully aware of all Valley Community Services Policies and Procedures and to perform work in accordance with such policies and procedures under the guidance of supervisors and professional staff
- All other duties as assigned
Qualifications:
- Valid PA driver’s license
- High school diploma or GED equivalent
- Background checks
- Physical/Mantoux TB required
- Pre-Employment Drug Testing
