NATURE OF WORK: This professional position provides both mental and general health care services to clients under the direction of a Physician by performing the essential duties of the job as listed below.
ESSENTIAL DUTIES:
- Examines, evaluates, and assesses patients and compiles patient medical data, including health history, labs, and results of physical and psychological examinations.
- Participates as a member of Valleys Assertive Community Treatment Team, Crisis Residential Unit Team, Alpha Chemical Dependency Treatment Unit, all of which includes both inpatient and outpatient adult care and the treatment of mental health, primary care, and substance use.
- Evaluates, diagnoses, treats, and manages patients including prescribing and administering medications, patient education, orders diagnostic tests and procedures and interprets tests results for deviation from normal, treating results and/or making referrals to specialists if necessary.
- Instructs and counsels clients regarding compliance with prescribed therapeutic regimens and disease processes, as well as educates patients on prescribed medications.
- Maintains complete, accurate, timely, and confidential documentation in an appropriate electronic medical health record, reporting to OSHA and Health Department as necessary.
- Develops, reviews, and implements client care plans along with treatment team to ensure the continuity of care in compliance with Valleys and State guidelines and physicians orders.
- Completes or arranges for completion of Patient Assistance Applications for indigent medications.
- Completes or arranges for completion of Prior Authorizations for treatment, procedures, and medications.
- Delegates tasks to appropriate staff as necessary.
- Participates as a member of the professional interdisciplinary treatment team as client needs dictate.
- Adheres to professional code of ethics in performing Nurse Practitioner services including Valleys Standards of Conduct and Compliance.
- Promotes appropriate community interactions by handling public interactions in a professional and competent manner. Returns telephone calls and requests for information in a timely manner, promotes a positive image of the Agency and maintains a good rapport with agency contacts.
- Attends in-service trainings and mandated meetings and serves on Committees as assigned.
- Supports and helps in the training of new employees.
- Performs basic, essential nursing duties, including but not limited to injections, venipunctures, nasopharyngeal swabs, wound cultures, etc.
- Handles psychiatric and medical emergencies as needed, including being on call.
- Collaborates with Medical Director regarding particularly complex conditions.
- OTHER DUTIES AS ASSIGNED
MINIMUM QUALIFICATIONS:
- Ability to perform Essential Duties as outlined below.
- Masters degree or higher in Advanced Practice Registered Nursing from an accredited institution of higher learning.
- Current valid license as a Nurse Practitioner in the State of West Virginia.
- Possess Prescription Privileges.
- Possess DEAX Waiver or willing to train to obtain DEAX Waiver.
- Knowledge of protocol, theories, principals and practices of a Nurse Practitioner.
- Ability to comply with Clients Rights and Confidentiality
- Ability to comply with Valleys and Division departmental safety procedures.
- Ability to read, write, understand and speak the English language.
- Valid Drivers License
