Job Description
The Clinical Outcomes Coordinator supports the Quality & Patient Safety Office through the abstraction of clinical data with the highest degree of integrity and creating reports that will transform data points into meaningful and actionable information. The Coordinator is key to providing information to Valley Health System leadership and providers to assist with optimizing clinical outcomes. The Coordinator works closely with providers and leaders to deliver routine and ad-hoc and to support process improvement opportunities. The Coordinator is also responsible for presenting and/or preparing presentation materials for a variety of groups including, but not limited to, service lines, quality teams, leadership teams, providers or other requested staff meetings.
The Coordinator abstracts clinical data from a variety of sources, including, but not limited to, EPIC, physician office notes and patient interviews. The Coordinator works independently to analyze the data to identify opportunities for improvement and assist with implementation of changes in clinical practices that will improve patient outcomes. The Coordinator demonstrates proficiency in maximizing potential of assigned database by attending annual meetings, actively participating on monthly database calls, and maintaining any registry certifications/requirements. The Coordinator also participates in regular calls or webinars within the national registry to maximize understanding of data definitions, outcomes reports, and notify the outcomes manager of any database changes. The Coordinator is an active participant in any committees, review boards and/or other compliance and regulatory boards as requested from Valley Health leaders. The Coordinator serves as an educational resource for other analysts, providers, and other members of the quality team to ensure that data definitions, analyzed reports, and clinical rationale be communicated properly.
Education
Bachelors Nursing (BSN) required
Masters preferred
Experience
5 years of clinical experience required
Basic knowledge of data interpretation and statistical analysis required
Strong analytical background with experience in clinical outcomes, benchmark data, and process improvement standards preferred
Informatics training preferred
Clinical informatics preferred
Surgical background preferred
Certification & Licensures
Licensed Registered Nurse or eligible for licensure in Commonwealth of Virginia required
