What are the responsibilities and job description for the Outcomes Coordinator position at Waterbury Hospital?
The Performance Improvement, Patient Outcome Coordinator leads and directs process improvement activities that provide efficient and streamlined workflow in the hospital environment. Presents results of improvement efforts and ongoing performance measures of clinical processes within the hospital to hospital leaders and WH in collaborative initiatives.
RESPONSIBILITIES:
Oversight of all quality measures for CMS.
Leapfrog coordinator
Coordinates the organization's effort to collect, analyze and evaluate outcome data through internal and external mechanisms.
Provides expertise on the needs for efficient data collection and analysis of measures used for reimbursement and comparison to other like facilities.
Provides education to staff and physicians concerning issues related to outcomes.
Uses critical thinking skills/activities to explore and develop initiatives designed to improve quality of care and patient safety.
Provides leadership and/or consultation to committees that impact process measures.
Provides team facilitation, education and support using recommended performance improvement tools and techniques to project teams as requested
Performs all other duties as assigned.
REQUIREMENTS:
Current Connecticut RN license.
Bachelor’s Degree in business or health related field preferred.
Ability to understand and interpret the organization's mission, policies, procedures, and services.
Experience in an acute care hospital setting.
Minimum of two years of experience in performance improvement
Demonstrated leadership skills
Ability to work throughout the organization with individuals in diverse areas and levels of responsibility.
Participation in community, state and professional organizations related to quality management and improvement
Computer proficient including internet use and management of multiple databases.
Ability to work independently.
Excellent communication skills
Strong analytical problem-solving skills.
Knowledge of hospital operations.
Excellent interpersonal and written communication skills
