JOB DESCRIPTION
CASE MANAGER
This job requires daytime, evening shifts and on call status.
Online Supervision:
- Ensure the safety and supervision of all residents.
- Review the logbook to stay informed about events since the last shift.
- Serve as a positive role model by demonstrating maturity, responsibility, and healthy behavior.
- Establish and maintain a therapeutic relationship with residents.
- Teach, model, and encourage social skills and appropriate social expression.
- Monitor and promote personal hygiene and proper appearance among residents.
- Transport residents to activities and appointments using Valley-Lake Boy's Home vehicles.
- Supervise daily schedules and oversee resident responsibilities, including chores, room cleaning, homework, community service, and summer work programs.
- Set and enforce appropriate boundaries for residents.
- Administer medications at designated times.
- Perform basic first aid procedures following certification.
- Determine and enforce consequences for rule violations.
- Implement and uphold resident policies and rules consistently and fairly.
- Organize and oversee mandatory recreational activities.
- Prepare meals when the cook is unavailable.
- Supervise residents during class transitions, meals, and while entering/exiting school buildings.
- Assist residents with schoolwork as needed.
- Answer phone calls and ensure messages reach the intended recipients.
- Supervise residents in public settings and off-site events.
General Paperwork:
- Document daily events, group attitudes, and behaviors in the logbook, including appointments, visitors, activities, meetings, and staff changes.
- Record daily evaluations of each resident’s behavior and attitude in individual behavior logs.
- Maintain logs for recreation, medication, community service, summer work programs, and special incident reports.
- Log new medications for residents in the medication log.
- Conduct initial intakes when the Executive Director or Program Director is unavailable.
- Track and log any money received by residents, securing funds in a locked money box.
- Make necessary copies of Valley-Lake Boy's Home forms.
Additional Expectations:
- Perform all duties professionally.
- Foster positive working relationships with co-workers, placing agents, and parents.
- Adhere to all Valley-Lake Boy's Home policies and procedures.
- Attend staff meetings and mandatory training sessions.
- Report maintenance needs or safety concerns to the Executive Director.
Case Management:
- Review residents' court orders to ensure compliance with all conditions and expectations.
- Establish initial contact with residents' families and placing agents, maintaining regular communication.
- Conduct initial assessments and develop individualized treatment plans.
- Coordinate necessary services and individuals for resident treatment.
- Communicate special accommodations or circumstances to staff.
- Monitor and facilitate residents’ educational needs, including school reintegration.
- Maintain regular contact with parents, keeping them informed about significant incidents and program updates.
- Address concerns from parents or placing agents in a timely manner.
- Understand and meet referring agents' expectations for each resident through ongoing communication.
- Conduct weekly sessions with residents and document progress.
- Prepare weekly progress reports for each resident.
- Organize staffing meetings at required intervals, including all involved parties, and prepare staffing reports.
- Maintain updated contact lists for residents.
- Summarize progress on treatment plans for staffing reports.
- Write release reports upon residents’ exits from the program.
- Schedule necessary medical, psychological, dental, and optical appointments.
- Contact the Valley-Lake Boy's Home chemical dependency agent if residents require treatment.
- Track court-ordered community service hours.
- Coordinate clothing allowances for residents in need and assist in purchasing appropriate clothing.
- Provide case note instructions for use during absence.
Qualifications:
- Must be at least 21 years old.
- Hold a bachelor's degree or higher in a human services-related field (e.g., criminal justice, psychology, social work).
- Pass a criminal background check.
- Maintain a valid driver’s license.
- Pass a drug screening.
- Be knowledgeable about local community resources and how to utilize them to support residents and families.
Job Type: Full-time
Pay: $22.00 - $24.00 per hour
