Valley Oaks Health, a local community mental health center located in Lafayette, Indiana, is searching for applicants to join our ever growing team. Position:
The Care Coordinator provides assessment, case formulation, diagnosis, crisis intervention/emergency services, and treatment planning to clients in the assigned service. They will coordinates with Office Managers and support staff to ensure quick access to referred services upon completion of initial assessment. Other job duties will include supporting the entire organization by conducting tele-assessments system-wide and handling all paperwork requirements and coordinating care for any additional activities/services the prescriber feels the client needs.

Who We Need:
1. Master’s degree in Social Work from a graduate school accredited by the Council on Social Work
Education and possesses Indiana License as a Clinical Social Worker.
2. Able to achieve the required competencies or privileges at Valley Oaks Health.
3. Valid Indiana Driver’s License and insurance, as applicable.
4. Meets pre-hire competency and reference requirements.
COMPUTER SKILLS:
Knowledge of spreadsheet software and word processing software such as Microsoft Word preferred.
KNOWLEDGE, SKILLS, AND ABILITIES:
1. Good usage of the English language; reading, writing, and speaking required.
2. High level of interpersonal skills to handle sensitive and confidential situations and documentation.
3. Attention to detail in composing, typing, and proofing materials, establishing priorities and meeting deadlines.
4. Good to excellent spelling, grammar, and written communication skills.
5. Excellent telephone and oral communication skills.
6. Ability to maintain a high level of confidentiality.
