RESIDENT CARE COORDINATOR

Reports To: Health Services Director
Purpose: The Resident Care Coordinator is responsible for the daily functioning of the health care needs of the residents in the Community. This includes supervising the resident assistants, supervising the medication delivery system, training of new staff, and servicing needs of existing staff in coordination with the Health Services Director. This position assists the health services leadership in the oversight of health services team members, procedures and systems. Universal Precautions, exposure to blood or bodily fluid is likely.
Physical Requirements (with or without reasonable accommodation):
*Ability to walk, reach and bend comfortably and frequently while moving through the Community and the ability to sit or stand for extended periods.
* Ability to push wheelchairs, ambulate residents and lift, push and pull up to 30 pounds routinely and up to 40 pounds occasionally.
* Fine hand coordination, and visual acuity.
Minimum Requirements:
1. Minimum of 1-year experience as a Caregiver or 6 months as a Medication Technician. Experience in Memory Care or Assisted Living setting preferred.
2. Ability to read written direction, comprehend verbal instructions, and write at a level necessary to accomplish job as well as the ability to communicate effectively with other staff members, supervisors, residents and visitors both verbally and in writing.
3. Ability to type at least 45 words per minute and have computer expertise at a level necessary to accomplish this job.

5. Self-motivated with the ability to work with little or no supervision.
6. Ability to work flexible hours to meet the requirements of the job.

Job Description & Acknowledgement Valley Spring Memory Care Duties & Responsibilities:
1. Partners with the Health Services leadership in the development and implementation of cooperative service plans and ensures that the cooperative service plans are completed and updated in a timely manner.
2. Performs various resident evaluations as assigned by Health Services leadership.
3. Assist in the planning, developing, organizing, implementing, evaluating and directing of resident care services.
4. Develop and maintain a good working rapport with inter-department staff as well as other agencies and businesses within the community to assure that the services and activities can be properly maintained to meet the needs of the residents.
5. Collaborates with leadership team to create and maintain Health Services schedule that accommodates requested days off, vacations and sick time, while maintaining appropriate staffing levels according to resident care needs and State requirements.
6. Coordinates information pertaining to changes in residents’ condition, personal needs, appointments, and outside services and communicates information to appropriate individuals.
7. Review and document daily in the communication log as appropriate.
8. Orders Health Services supplies as needed, maintaining necessary inventory for proper resident care, communicates and partners with Executive Director to track spending and stay within budget.
9. Receive verbal orders from physicians, physician’s assistants and nurse practitioners and chart on the physician’s order sheet. Review new orders and MAR daily.
10. Supervise and review the transcription process, medication and equipment ordering, and pharmacy cycle fill.
11. Perform resident care coordination and communication with both outside agencies and medical providers.
12. In the event of a resident death, may call the funeral home as indicated in the resident information and as otherwise outlined in the policy and procedures of the Community.
13. Review alert charting and provide appropriate follow-up.
14. Make written and oral reports and recommendations to the Health Services Director and Executive Director concerning the operations of the department.
15. Review incident reports as needed.
16. Assure that the Medication Room is maintained in a clean and safe manner for residents’ comfort and convenience by assuring that all necessary equipment and supplies are maintained to perform such expected and intended.
17. Review, supervise and assist with resident care as needed.
18. Performs medication pass as needed.
19. Assists residents with meals as needed.
20. Review and document on the service plan as needed and attend service plan meetings review as needed.
21. Reviews and responds to the Health Services Director’s audit to ensure compliance. 22. Meet with caregiving staff, as assigned by the Health Services Director, to assist in identifying and correcting problem areas and/or the improvement of services.
23. Coordinates and assists in interviewing, hiring, onboarding and training for all Health Services team members.
24. Schedule the exit-training interview with terminating employees.
