Join Our Team at Valley-Wide Health Systems, Inc.
We are seeking a compassionate and detail-oriented Community Support Specialist to join our team. This position is responsible for managing and monitoring day-to-day patient caseloads and providing Community Support Services to help individuals coordinate care, overcome barriers, and improve their overall well-being.
Key Responsibilities:
- Provide care coordination and life skills training for patients with significant mental illness.
- Manage caseloads for both Community Support Services and Meds Only Program patients.
- Develop, implement, and monitor individualized treatment plans.
- Conduct assessments, document patient interactions, and maintain accurate clinical records.
- Coordinate care in clinic settings, patient homes, and community environments.
- Schedule and attend appointments for Restoration Program patients, if assigned.
- Collaborate with clinical teams to support patient care goals.
- Attend team meetings, individual supervision, and group trainings as scheduled.
- Ensure timely documentation (within 48 hours) and meet productivity and compliance standards.
- Demonstrate cultural competence and sensitivity in all interactions and documentation.
- Participate in secure patient transports when needed.
- Uphold HIPAA requirements and maintain strict confidentiality at all times.
Qualifications & Skills
Required:
- High School Diploma or equivalent.
- Lived Experience – Personal history of recovery from addiction, mental health condition, trauma, or caregiving for someone with such conditions.
- Certification as a CPFS, NCPRSS, or equivalent (or ability to obtain within 1 year of hire).
- Willingness to obtain a Bachelor’s degree within a designated timeframe after hire.
OR
- Bachelor’s degree (preferred) in Social Work, Counseling, Psychology, or related field.
- Candidates with non-related degrees may be considered with one year of relevant experience.
Additional Qualifications:
- Valid Colorado driver’s license and ability to travel as needed.
- Ability to pass a Behavioral Health background investigation.
- Strong written and verbal communication skills.
- Proficient in Microsoft Office and general office equipment.
- Demonstrates empathy, professionalism, and cultural sensitivity.
- Excellent time management and organizational skills.
- BLS Certification (provided upon hire).
- Team-oriented with a collaborative mindset.
