Community Support Specialist

We are seeking a compassionate and detail-oriented Community Support Specialist to join our team. This position is responsible for managing and monitoring day-to-day patient caseloads and providing Community Support Services to help individuals coordinate care, overcome barriers, and improve their overall well-being.

Key Responsibilities:

• Provide care coordination and life skills training for patients with significant mental illness.
• Manage caseloads for both Community Support Services and Meds Only Program patients.
• Develop, implement, and monitor individualized treatment plans.
• Conduct assessments, document patient interactions, and maintain accurate clinical records.
• Coordinate care in clinic settings, patient homes, and community environments.
• Schedule and attend appointments for Restoration Program patients, if assigned.
• Collaborate with clinical teams to support patient care goals.
• Attend team meetings, individual supervision, and group trainings as scheduled.
• Ensure timely documentation (within 48 hours) and meet productivity and compliance standards.
• Demonstrate cultural competence and sensitivity in all interactions and documentation.
• Participate in secure patient transports when needed.
• Uphold HIPAA requirements and maintain strict confidentiality at all times.

Qualifications & Skills

Required:

• High School Diploma or equivalent.
• Lived Experience – Personal history of recovery from addiction, mental health condition, trauma, or caregiving for someone with such conditions.
• Certification as a CPFS, NCPRSS, or equivalent (or ability to obtain within 1 year of hire).
• Willingness to obtain a Bachelor’s degree within a designated timeframe after hire.

OR

• Bachelor’s degree (preferred) in Social Work, Counseling, Psychology, or related field.
• Candidates with non-related degrees may be considered with one year of relevant experience.

Additional Qualifications:

• Valid Colorado driver’s license and ability to travel as needed.
• Ability to pass a Behavioral Health background investigation.
• Strong written and verbal communication skills.
• Proficient in Microsoft Office and general office equipment.
• Demonstrates empathy, professionalism, and cultural sensitivity.
• Excellent time management and organizational skills.
• BLS Certification (provided upon hire).
• Team-oriented with a collaborative mindset.
